Section 12.1
Sedative-Hypnotic & Antianxiety Drugs
Compare GABA-A modulators with buspirone and evaluate sedation, dependence, withdrawal and reversal risks.

Idea 1 / 3
Benzodiazepines amplify inhibition
Benzodiazepines bind an allosteric site on GABA-A receptors and increase the frequency of GABA-mediated chloride-channel opening. They require GABA; they do not directly open the channel like an on-off switch.
Anxiolysis and impaired coordination arise from the same increased inhibitory signaling in different circuits.

Sedative effects add together
Alcohol, opioids and other CNS depressants can combine with benzodiazepines or Z-drugs to deepen sedation and respiratory compromise. Complex sleep behaviors are an important Z-drug warning.
A familiar prescribed dose can become dangerous when combined with an opioid or alcohol.
Dependence and alternatives
Repeated benzodiazepine exposure can produce tolerance and physical dependence; abrupt withdrawal can cause severe symptoms including seizures. Buspirone acts mainly through 5-HT1A partial agonism, has delayed anxiolytic benefit and does not substitute for emergency benzodiazepine withdrawal management.
Buspirone is unsuitable as an “as-needed immediate sedative,” but may fit a planned chronic anxiety strategy.
Common misconception: Flumazenil can reverse selected benzodiazepine effects, but it is not a universal antidote: it can precipitate withdrawal or seizures and does not reverse opioid toxicity.

A GABA-A channel can calm circuits and impair safety
Compare receptor modulation with a non-GABA buspirone route, and keep sedative stacking and withdrawal separate from symptom relief.

Go deeper: worked example
A person taking a benzodiazepine regularly receives an opioid and becomes difficult to awaken with slow breathing.
- Recognize additive CNS and respiratory depression as an emergency.
- Support airway and ventilation and identify all substances.
- Do not assume flumazenil is automatically safe: dependence, seizure risk and mixed overdose affect the decision.
Reversal is a risk-benefit decision within resuscitation, not a substitute for supportive care.
Explain it yourself
Why can the same GABA-A action relieve anxiety and impair driving?
Reveal explanation
Enhanced inhibition affects anxiety circuits but also alertness, reaction time, memory and coordination.
Create and evaluate
Write a safety plan for starting or stopping a sedative-anxiolytic.

Compare with the rubric and example
- Links the selected drug to its receptor mechanism and onset.
- Addresses driving, alcohol, opioids and other depressants.
- Distinguishes gradual supervised withdrawal from emergency overdose care.
Confirm the goal and expected onset, test individual sedation before driving, avoid additive depressants, never stop sustained benzodiazepine use abruptly, and treat unresponsiveness or slow breathing as an emergency.
